Continuing Education and former students (or current students that also attended prior to 1995) will need to

to have your form completed and returned to you before attaching to your Transcript request.
Transcripts are sent as-is. Please check to ensure your degree and/or most recent grades have been posted before ordering your transcript.
There is a $7 fee for electronic transcripts. Mailing options are available with additional fees.
